Movie Assessment Summary General Instructions: Select a movie with at least one character that portrays a mental illness. You will need to get your film approved by me no later than Sunday, September 10. This should be done on the Blackboard Discussion Board and will count for 5 points on your paper. Your paper will be due by Sunday, September 17. Choose a disorder that you find interesting and want to learn more about. Pay attention to identifying information, family history, social history, and psychiatric symptoms with example scenes from the film. APA Format and Style: Use APA (7th ed) format in this paper. This includes: • Times New Romans, 12-point font (black ink) • 1-inch margins on all four sides • Double-spaced • Title page • Levels 1 & 2 headings • References page that corresponds to citations in your paper • All quotations cited correctly, including page number. Use the textbook as one of your sources. Use the following format called “Assessment Summary” for your paper: Assessment Summary of Name of Film (notice that the film name is in italics) Identifying Information • Name • Age • Race • Sex • Brief statement presenting the problem. Example: John Smith is a 42-year-old white male who presented with a history of chronic substance abuse. Family History • Information about biological parents • Information about caretakers (if different from natural parents) • Information about siblings • Information about family as it relates to the presenting problem. Example: The client reported that his biological parents reared him: Bob Smith who is 69 years old and Mary Smith who is deceased. The client has three siblings: Ron (age 38), Tabitha (age 40) and Bubba (age 45). The client stated that his mother died of liver complications four years ago and that his brother, Ron, is a recovering drug addict. Social History • Educational level • Academic progress • Relationship with peers growing up • Other significant information that relates to the presenting problem Example: The client stated that he completed the seventh grade and later obtained a GED. His grades in school were poor (Cs, Ds, and Fs). He stated that he always had many friends; however, all of his friends were drug abusers and were in chronic trouble with the law. The client stated that he was held in a juvenile detention facility for shoplifting when he was 16. Additionally, he also stated that his driver’s license was suspended after receiving three DUIs in the span of three years between 37-39 years of age. Presenting Problem Re-state the problem. You should discuss frequency, intensity, duration of symptoms, etc. Also, other significant contributors; essentially, all information gleaned from functional analysis stated as concisely as possible. Example: The client reported a 24-year history of chronic substance abuse. Abused Substances include alcohol, marijuana, cocaine, amphetamines, and heroin. The client is currently on a methadone maintenance program at Light Memorial Hospital; however, he continues to use all the other drugs mentioned. He stated that he begins to use marijuana upon awakening, generally, at approximately 1 pm; he begins to drink beer shortly thereafter. He stated that he typically drinks 1.5 cases of Rolling Rock beer, or a fifth of Johnny Walker Red Label Scotch, before 5-6 pm. He uses other drugs when available, typically once or twice per week. Charts revealed that he was hospitalized twice for drug addiction with no success in maintaining abstinence. Charts also state that he has had several substance-related legal problems including three DUIs. A medical examination revealed a deficiency of Vitamin B12, which is common with chronic alcoholics. Assessment A summary statement of your assessment findings followed by a DSM diagnosis. Describe each symptom found in the DSM-5, quoting from this source, and clearly linking the information to examples about the character in your movie. Remember to quote and cite the DSM-5 according to APA format. Treatment Goals This section should contain a complete listing of treatment goals, addressing each diagnosis. The goals should be clearly related to the information presented at this point in the paper. You will need to research appropriate treatments and consider relevant goals for the client, which may include types of medications, specific individual therapy models shown to be effective for each diagnosis, support groups, family therapy, health-related goals relevant to the diagnosis, relational goals necessary for recovery, etc. Use at least three academic sources (peer-reviewed research journal articles or scholarly books) to back up your treatment goals; your text may be one of those sources.
