Mr. W an 82-year-old man residing in a long-term care facility. he has a history of congestive heart failure, hypertension, and dementia. Mr. W is frequently anxious, particularly when being transferred to and from a wheelchair. his gait is unsteady and his lower extremities are weak. Mr. W performs basic self-care (oral hygiene, face washing), but needs assistance with toileting and complete bathing. he is able to feed himself. he is encouraged to be as independent as possible, but he is very slow to do so.
