MA Scenario -Revised

Attached paper need s to be revised and corrected. Be sure to use correct English, spelling and grammar. Sources must be cited in APA format to included cited sources in text body. Your response should be four (5) double-spaced pages. NO PLAGIARISM!

Part A: Kayla Christianson, CMA, has been employed six years by the cardiology practice of three physicians. She is a graduate of a CAAHEP-accredited school. Furthermore, Kayla received extensive hands-on training performing ECGs while doing her required externship.

Kayla has completed an ECG ordered by Dr. Hsu for Mrs. Warner, a 76-year-old patient. Dr. Hsu, Kayla’s boss, has telephoned her explaining that he was behind schedule doing rounds at the hospital. He asked her to do him a favor and interpret Mrs. Warner’s ECG, sign his name, and fax the report to Mrs. Warner’s referring internist who is expecting the results.

Provide detailed answers for each of the following questions.

Your response should be two (2) pages in length.

· Given the scope of Kayla’s education, training, and years of experience as a CMA, would this favor fall within the AAMA guidelines of her responsibilities?

· Would any portion of Dr. Hsu’s request fall within the guidelines? If so, which portion(s)? Is an exception to these guidelines ever allowed?

· How should Kayla respond to Dr. Hsu?

Part B: For more than two years, medical assistant Linda Lewis had been employed by Drs. Norek and Klein, who are gerontologists. Also on staff are two registered nurses, a medical laboratory technician, and a medical social worker. The daughter of one of the doctor’s patients has just called the office. She is very distraught at the seemingly diminished capacity of her mother and insists on speaking to the doctor.

Linda explains that both physicians take only emergency calls during patient appointment hours, but that she will take a detailed message. This caller, however, suggests that not only should her call be considered an emergency, but that she will sue the doctor if the call is not handled accordingly.

Provide detailed answers for each of the following questions.

Your response should be two (2) pages in length.

· What should Linda do immediately to diffuse the situation?

· Is this clearly a case when the call should be passed on to one of the registered nurses or the medical social worker?

· Is this a case when the physician should be called to the telephone because of the threat of an impending suit?

· How could Linda ascertain whether or not this is indeed an emergency? Is it up to her, as a medical assistant, to make such a determination?

· Because this is the patient’s daughter rather than the patient herself, does Linda have any reason to enter into a conversation with the caller? Could Linda be ethically bound by confidentiality not to admit the woman’s mother is a patient?
MEDICAL ASSISTANT STAFF 6

Duties and Responsibilities of Medical Assistant Staff According To American Association of Medical Assistants (AAMA) Guidelines

Part A

Certified Medical Assistant (CMA) implies that as clinical support, one can perform all roles suitable for medical practice and not limited to injection administration, ordering medical supplies, helping the staff physician, blood drawing, assist with other office works as required and offering education to patients. There are so many questions regarding CMA experience necessities and whether one’s job is deemed eligible for the certification. The qualifying experience quest for a position that needs judgment and occasionally employing principles of clinical management. This certification requires proficiency where the applicant must qualify some of the requirements to train for the certification. Considering Kayla is training at for a CMA, it means that she had passed the requirements for the certification. Among the requirements include portable EKG testing experience; the function of the electrocardiogram is to measure the electoral activity of the heart from how fast the hearts beat to its efficiency of the chambers to conduct electrical energy (Lindh et al., 2013). The supporting clinical staff should also be motivated and flexible. An EKG assists physicians diagnose emergencies such as a regular heart attack and where one has high pressure the EKG shows the enlargement of the heart as a result of extra pumping of the blood.

American Association of Medical Assistants (AAMA) is the sole medical assisting certification requiring graduation from postsecondary medical assisting program certified by a certification body acknowledged in the United States. AAMA aims to offer the medical assistant with certification, credential acknowledgment, support for quality patient health care and education. According to AAMA, there are several responsibilities that its associates it medical supporting staffs which include administrative duties and clinical duties. As a clinical duty, a medical supporting is supposed to perform electrocardiograms and preparing medications as directed by a physician (Blesi, 2016). Under the above scope, a Kyla is only supposed to perform electrocardiograms but not explain to them as directed by the physician. First, it’s the duty of the Kyla to answer telephones that she did by answering Dr. Hsu phone call as it lies under the responsibility of the medical assistant. As per the document AAMA guidelines, a medical assistant is only supposed to perform duties that are in line with his or scope of work. Dr. Hsu telling Kyla to interpret the electrocardiograms result is beyond Kyla scope of work, and she has no right to interpret the results. Although there is a clinical duty that Kyla is supposed to perform in assisting the physician during exams, interpretation electrocardiograms go beyond her scope of work as it is not documented as part of the assisting duties. Interpretation of electrocardiograms results is very critical as it requires a specialist to do so like Dr. Hsu as the interpreted results will help a cardiologist determine if some parts of the heart are overworked or enlarged. Kyla is supposed to respond to Dr. Hsu by rejecting the duty to interpret the results. Dr. Hsu should understand the complexity of the analysis and that the results require qualified personnel for the cardiologist to make the accurate conclusion of Mrs. Warner heart problem. In case the results are misunderstood, the cardiologist may also offer the wrong conclusion of the heart problem leading to wrong medication. This may hold Dr. Hsu responsible and liable in the AAMA guidelines for directing Kyla to interpret the electrocardiograms which are beyond her scope of clinical duties.

Part B

Most incensed customers some of the time expects one to think before expressing what ought to be done next. Give them that time perfect from the soonest beginning stage – successfully tuning in to the discord so you can get to the fundamental issue and a possible course of action. Try not to Answer Emotion with Emotion; It is anything but difficult to get enthusiastic when someone gets angry with you. In any case, the enraged customer on the opposed side of the counter or phone line is likely not unglued at you – yet somewhat incensed at your association.

What Linda ought to have done as soon she got the telephone call to diffuse the circumstance was influenced the little girl’s patient to feel good, and she completely well took administer to the medicinal staff alongside question to realize what was the crisis and exchange the call to one of the enrolled attendants for prompt help. Additionally, a way for Linda to counter a high volume from a customer is a low one that seepages calm and resilience. By a comparable token, watch your tone when you are responding to an incensed customer.

Indeed, it is unmistakably for this case this call ought to pass it into one of the enlisted nurture since they are the one with the learning who can give the girl’s patient the main right hand on what to do amid her crisis, while the specialist could restore her call. Truly they should pass it on to the therapeutic social laborer since they are knowledgeable about such circumstances.

Indeed, the specialist ought to be quickly told about the call and how the little girl ‘s persistent felt and her charges of sue if her call isn’t viewed as a crisis and dealt with as needs are of the circumstance. Indeed he should come because a claim is something intense and may cause lose the notoriety of the place.

The main route for Linda to know regardless of whether this is a crisis, she ought to ask the little girl’s patient guide inquiries to partner distinguish the kind of crisis and how the patient could help. Linda as a therapeutic collaborator has the experience and the learning to understand how to deal with and decided whether the patient life is in peril. Truly because she has experienced appropriate preparing as a medicinal preparing and being allocated to accepting calls needs a man who is very much educated and can detect a crisis from afar.

Indeed, as indicated by HIPPA if the patient can’t talk and their life is in peril anybody can look for their therapeutic consideration too to know the determination of the patient. For this situation, Linda ought not have any moral or classification results for conceding or drew in a discussion with the patient’s girl on the grounds that the patient most likely was not in condition to talk for herself and furthermore if the little girl called implies she was at that point approved by the patient to settle on any choices about her mom. Like a little girl she has the privilege to know however Linda ought not to uncover everything to her in spite of her being the little girl. She should tune in and comfort the kid and guarantee her that there is no reason to worry.

Healthcare Policy and the Nursing Home Industry

Order Description
It is important to understand the process of how ideas become regulations and rules and how rules
and regulations become policy and laws for the nursing home industry. Imagine yourself as a public
speaker trying to simply explain this path on the local, state, and national level to a bipartisan group of nursing home administrators. Your explanation should show the story of how healthcare policy is
produced and how that policy affects the nursing home industry.

Instructions
Create a visual depiction using any tool you wish (graphs, charts, figures, pictures or PowerPoint) to
explain the process of how ideas become regulations and rules and how rules and regulations
become policy and laws on the local, state, and
national levels for the nursing home industry.
Research the path of how local, state, and national rules, regulations and laws are created and
include the knowledge you find from this research into the visual depiction you create. The depiction
should be concise and easy to understand. For example, no more than five PowerPoint slides if
PowerPoint is used or no more than 2 pages of graphs, charts, figures or pictures if that option is
used. APA is not required except for the references section.

Requirements
? Writer visually depicts a clear and easy to understand path of how ideas become healthcare policy on a local, state and national level.
? Writer visually depicts how individuals or groups can impact this process and their role in the
process.
? Writer visually depicts how the process impacts the nursing home industry.

structuer biology

The main goal of this assignment is two-fold:

1. Enable you to critically read and understand a structure paper and apply what you have learnt so far.
2. To acquire expertise in using the various structural tools/resources you have learnt in the course to a protein-ligand complex of your choice. This will also serve as building blocks for your final project. If you choose your molecule wisely, you can get a head-start on your final project.

(Total: 30 points, will account for 15% of your final grade)

Please upload your assignment directly to Blackboard. DO NOT e-mail the assignment. Please submit by the due date/time. LATE ASSIGNEMENTS AND ASSIGNMNETS E-MAILED TO ME WILL BE SUBJECT TO A PENALTY.

Some rules to follow:

You must choose your own protein-ligand complex No duplicates.
Make sure your answers are brief and to the point
You are welcome to discuss your doubts with me at anytime
HOWEVER, you MUST COMPOSE AND TYPE IN your final answers yourself
AND you are expected to be able to explain the basis of your answer Please DO NOT copy sentences from the article. Rephrase it in your
own words.
DO NOT COPY from your friends. YOUR ASSIGNMENT WILL NOT BE GRADED.

Keep in mind that presentation is an important aspect of communication in Science. Bonus points are possible.

SECTION 1:

The main purpose of this section is to read and understand the differences between X-ray and NMR methodologies. Read the following paper and answer the questions that follow:

Structure of the interleukin-2 tyrosine kinase Src homology 2 domain; comparison between X-ray and NMR-derived structures.
Joseph RE et al. Acta Crystallogr Sect F Struct Biol Cryst Commun. 2012 Feb 1;68(Pt 2):145-53. Epub 2012 Jan 25.PMID:22297986. The paper has been uploaded to Blackboard.

1. Summarize in not more than four to five sentences the main goal and the rationale for undertaking structural studies of the interleukin-2 tyrosine kinase Src homology 2 domains. What are the PDB-Ids of the structures described in this paper? (2 point)
2. Answer the following questions based on the crystal structure: (4 points)
a. What methodology (for phasing) was used to solve the structure? Experimentally what changes had to be made to the wild-type protein that enabled the authors to crystallize and eventually solve the structure by the described methodology and why did the authors have to make this change? In other words, couldn’t they have solved the wild-type structure without this change?
b. Summarize in the form of a table the following: (DO NOT CUT &
PASTE the Table from the paper)
(i). Resolution of the final refined structure
(ii). R-factor and R-free
(iii). Data Completeness
(iv). Average Temperature Factors (this reflects the dynamics and not the temperature at which the experiment was conducted)
(v). Using one of the tools you have learnt in class provide a snapshot of the Ramachandran plot of the crystal structure. What % of the residues is in the disallowed region of the plot if any?
(vi). Based on all the crystallographic parameters above, is the structure reliable? Does it support the author’s claims? Describe your answer in not more than 3-4 sentences. You can use bullet points.
3. What kind of NMR spectra was used to solve the structure? Provide the experimental details used for the NMR experiment including the instrumentation used. Provide a snap-shot of the NMR spectra of WT and mutant SH2 (you are welcome to take this from the paper by providing a
reference). (2 point)

4. Summarize in 4-5 sentences the major differences if any, between the Xray and the NMR structures. Summarize in 2-3 sentences what major conclusions were made from each of the two structures? Was there agreement between the two structures? Which methodology is yielding more information to the authors and why? (You may tie this to the rationale behind this paper). (4 points)

5. What were the major unresolved questions that the NMR and X-ray structures described in this paper help to address? In one or two sentences, comment if you think these structures are going to further our understanding of the role of cis-trans isomerization in proteins. (3 points)

SECTION 2:

Using Heteroatom navigator
(http://hetpdbnavi.nagahama-i-bio.ac.jp/index.php?mode=0) or

PDB web-site (http://www.rcsb.org/pdb/home/home.do) identify a structure of a complex determined either by X-ray crystallography or NMR with your favorite ligand bound (small molecule). Make sure that the ligand is a biologically relevant one! If you have questions about this, please let me know. Your assignment will not be graded if you choose a ligand that is not physiologically/biologically relevant. For example acetate, glycerol and azide are not biologically relevant in crystal structures. Examples of biologically relevant molecules are ATP, GTP, GDP, NAD, NADP, NADH, PLP, FMN, FAD, SAM etc. This list by no means is exhaustive.

Before you proceed make sure that your structure has electron density data in the EDS server and is biologically relevant.

1) Write out the PDB-ID and the PMID associated with the structure you have chosen. Write a few sentences about the biological function of the protein-ligand complex structure you have chosen. Not more than 4-5 sentences please! DO NOT COPY sentences from the paper. This is considered plagiarism (3 points)

2) Tabulate (present as a table) the H-bond interactions that the ligand makes with protein using PDBSum and Ligand Explorer (provided within the structure record in PDB) tool/resource. Please include both the ligand atoms and the protein residues involved in the interaction.

Is there any bridged water molecules involved in stabilizing the complex? (you can get this information using the Ligand Explorer tool provided within the PDB record). If yes, what residues is it bridging? If no, just mention that there are no bridged water molecules.
Do not just provide a snap-shot of the interactions! (6 points).

Example:
GDP
(Ligand) Atoms Amino Residue Acid Hydrogen Bonds Avg. Distance (Å)
N7 Asn 116 A 1 3.10

3) Write-out any interesting structural features seen using ProMotif (for example, does it have any 310 helices or gamma turns etc.). If none of this exist, list the ones you see.

Provide a snapshot of the Ramachandran plot for your chosen structure.
Comment about the stereochemistry based on the plot in one sentence.
(3 points). For example, is the structure reliable?

4) Classify your protein-complex using the SCOP database. Provide fold, superfamily, family and class information. If SCOP does not contain the information for your structure, please use the Superfamily database.

Biostatistics in healthcare

Immune Responses Abnormal immune responses can trigger a range of autoimmune diseases, in which an individual’s immune system is attacking normal tissues in the body. Well-known examples of autoimmune diseases are type 1 diabetes mellitus, lupus, and multiple sclerosis. Ideally, one would like to harness the immune system to attack abnormal substances or tissues like cancer, while sparing the normal (unaffected) tissue. Many tumor cells produce antigens (proteins) that theoretically ought to trigger an immune response: that is, one’s immune system ought to recognize cancer cells as somehow foreign or abnormal, and thereafter eliminate these cells from the body. The field of cancer immunotherapy is actively pursuing this study. Tumor antigens may also be useful for diagnostic tests; high levels of tumor antigens could be taken as markers or indicators of cancer. In this assignment, you will be examining levels of tumor-associated antigens (TAAs) as determined from immunoassays (i.e., biochemical tests that measure the concentrations of the tumor-associated antigens in serum samples).

Download the Excel file MHA610_assignment_3_data.xls, and open it.
The spreadsheet contains data on 250 individuals: 90 normal individuals from San Diego (the controls), and 160 individuals from Korea and China, all of whom were diagnosed with hepatocellular carcinoma (HCC).
Serum samples were taken from the controls and from the cases at time of diagnosis of HCC. Levels of a panel of 12 tumor-associated antigens (TAAs) were assessed via immunoassays in all individuals;
The levels are given in the columns with headings Ab14, HCC1, IMP1, KOC, MDM2, NPM1, P16, P53, P90, RaIA, and Survivin. (These are the designations of the 12 TAAs, all of which were thought to be potentially predictive of cancer.)
The underlying question is whether we can effectively discriminate between the cases and controls on the basis of the levels of these TAAs. This is sometimes termed a classification problem in the statistics and biostatistics literature: we wish to classify individuals as normal or cancer patients on the basis of their TAA levels.
We will examine these data in Statdisk. Use the MHA610_Week 3_Assignment_Data.CSV file to upload this information into Statdisk.
If you choose the latter option, Start Statdisk, then choose File>Open and select the .csv file you created (unless you changed the name, it ought to be MHA610_assignment_3_data.csv)
Check the box that specifies the data contains column titles or headers, select Comma separated for how the data are delimited, click finish, and the dataset will have been successfully imported into Statdisk.
NOTE: you may want to read through the remainder of the assignment first, before proceeding with this step. This may save you some work afterwards!
Note that Statdisk operates on columns of data, and that both cases and controls are contained in each column of TAA levels. It will be necessary to separate the cases and controls for further analyses. This can be accomplished either by copying within Statdisk or by reverting to the original Excel workbook, copying in Excel, exporting as a .csv file, and then importing into Statdisk. (Don’t say you weren’t warned!)
Explain if you would characterize any or all of the TAA levels as approximately normally distributed for the controls and for the cases.
Provide plots and statistics in support of your conclusions.
Explain if any of the TAAs are useful for discriminating between the cases and controls.
Provide plots and statistics in support of your conclusions.
All writing assignments should be at least 250-500 words in APA format supported by scholarly sources.
BONUS. In the above, we pooled all cases together. Summarize whether you think this is legitimate or whether the levels of any of the TAAs appear to differ significantly between the cases from China and the cases from Korea. Provide evidence in support of your conclusion.
