Discussion Post

A} Dissemination of EBP and research, such as presenting results at a conference or writing an article for a journal, is an important part of professional practice. Identify one professional journal and one nursing or health care conference where you might present your project. Discuss why each of your choices is the best option for you to disseminate your new knowledge.

B} Discuss why EBP is an essential component of the practice of a BSN-prepared RN. Identify two ways in which you will continue to integrate evidence into your practice and encourage it within your work environment. What obstacles could challenge this plan, and what steps will you take to minimize their impact?

Internal Environmental Analysis/Strategy Analysis And Strategy Selection

Step 1: Research

In completing the report, students will use the chapters in the eBook as a guide and perform research on the company from Assignment 1, answer the required elements below in narrative form following the steps.

Library Resources

You will be using scholarly material and Mergent Online to research privately held companies. On the main navigation bar in the classroom select, Resources and then select Library. Select Databases by Title (A – Z). Select M from the alphabet list, and then select Mergent Online.

UMUC library is available for providing resources and services. Seek library support for excellence in your academic pursuit.

Library Support

Extensive library resources and services are available online, 24 hours a day, seven days a week at https://www.umuc.edu/library/index.cfm to support you in your studies. The UMUC Library provides research assistance in creating search strategies, selecting relevant databases, and evaluating and citing resources in a variety of formats via its Ask a Librarian service at https://www.umuc.edu/library/libask/index.cfm.

Step 2: Company Selection

The companies used in Assignment 1 are below. Students will use the same company to complete this project as they did in Project 1. Students who fail to use the companies on the list or use an unapproved company will receive a zero for the assignment.

Ball Corporation [NYSE: BLL]- My company from week 1( use this company)

Michael Kors Corporation [NYSE: KORS]

National Beverage Corporation [NASDAQ: FIZZ]

YUM China Holdings, Inc. (YUMC)

Step 3: Preparation for the Assignment

Before you begin writing the report, you will read the following requirements that will help you meet the writing and APA requirements.

You will be doing an analysis on the selected company. When doing an analysis you are not merely making statements that may be cited. Instead, you will be supporting the statements made. “Support” is the process of explaining, discussing and analyzing “why” and “how,” which is a higher level critical analytical skill that is required. Support is needed to do well on this project.

Read the grading rubric for the assignment. Use the grading rubric while writing the report to ensure all requirements are met that will lead to the highest possible grade.

Step 4: Introduction

Create an introductory paragraph. The Introduction should clearly and concisely convey the main points of the assignment’s requirements.

Step 5: How to Set Up the Report (Ensure these are followed)

Create a Word or Rich Text Format (RTF) document using 12-point font. The final product cannot be longer than 16 pages in length, which includes all tables and matrices but excludes the title page and reference page. Those items identified in the technical analysis should appear under the appropriate heading in the paper. Do no use an Appendix.

Follow the following format using these topics as headings:

Title page with title, your name, the course number, the instructor’s name;

Company Background/Information

Internal Environmental Analysis

Strategic Analysis & Strategy Selection

Reference page

Step 5: Required Elements for the Report

Under the appropriate heading listed above, include the following:

Background analysis including vision and mission statements and objectives

Internal Environmental Analysis

Corporate Level Strategy

Business Unit Level Strategy

Functional Level Strategy

Explain how these strategies align with the company’s vision and mission statements;

Assess the company’s interactions with its stakeholders, the organizational structure, the organizational culture, and communication/decision making among managers within human resources, marketing production, operations, finance and accounting, R&D, and computer information systems, which can be accomplished by viewing the company’s website, interviews, and surveys.

Financial analysis for the last reported fiscal year

Use the company’s income statement and balance sheet to calculate no less than a total of ten (10) key financial ratios to the business. There must be a mix of four different key categories inclusive of the leverage, liquidity, profitability, and efficiency ratios so that the ratios do not all come from the same category. The specific ratios selection must come from the following categories.

Leverage Ratios (Long term debt ratio, Total debt ratio, Debt-to-equity ratio, Times interest earned ratio, and Cash coverage ratio).

Liquidity Ratios (Net working capital to total assets ratio, current ratio, quick ratio, and cash ratio)

Efficiency Ratios (Asset turnover ratio, Average collection period, Inventory turnover ratio, and Days sales outstanding)

Profitability Ratios (Net profit margin, Return on assets, and Return on equity)

Technique Analysis

Develop and explain an IFE, BCG matrix, Grand Strategy Matrix, and QSPM. The expectation is not to copy from the Internet but to develop one’s own. If copied from the Internet, a zero will be assigned. The various tools are to appear in the appropriate area of the paper and not in one section of the paper. When placing any table or figure in a table, it must be explained in detail.

Strategy Analysis

Identify and explain company strengths and weaknesses. Discuss success factors and what the company must do to perform successfully in the industry? Discuss what strategies would allow the company to capitalize on its major strengths. Discuss strategies that would allow the company to improve upon its major weaknesses

Alternative Strategies

Generate a minimum of three possible alternative strategies for the company;

Identify and discuss cultural factors that should be considered in analyzing and choosing among the alternative strategies;

Prioritize and explain the selection of alternative strategies

Recommendation

Recommend the best one or two strategies among the alternative strategies and explain why these strategies are the best;

Step 6: Report Requirements to Follow

In writing the report,

Use the grading rubric while completing the project to ensure all requirements are met that will lead to the highest possible grade.

Third person writing is required. Third person means that there are no words such as “I, me, my, we, or us” (first person writing), nor is there use of “you or your” (second person writing).

Contractions are not used in business writing, so do not use them.

No direct quotes except for mission statement and vision statement. For all other source material used in the analysis, you will not use direct quotation marks but will instead paraphrase. What this means is that you will put the ideas of an author or article into your own words rather than lifting directly from a source document. You may not use more than four consecutive words from a source document, as doing so would require direct quotation marks. Changing words from a passage does not exclude the passage from having quotation marks.

Use in-text citations and provide a reference list that contains the reference associated with each in-text citation.

You may not use books in completing this project.

Provide the page or paragraph number in every in-text citation presented.

Discussion 10

According to the CDC, “in the United States, the proportion of the population aged >65 years is projected to increase from 12.4% in 2000 to 19.6% in 2030,” (CDC, 2003, para. 2). Caring for this aging population is going to be one of the greatest challenges facing the health care industry. Not only will the number of individuals requiring care rise, but so will the cost. As poignantly stated by Crippen and Barnato, “unless we change the practice of medicine and reduce future costs, and explicitly address the ethical dilemmas we face, there may come a time when our kids simply cannot afford us” (2011, p. 128).

In this Discussion, you will examine the ethical issues that the United States and other nations must address when faced with the health care challenges of an aging population.

To prepare:

Consider the ethical aspects of health care and health policy for an aging population.
Review the Hayutin, Dietz, and Mitchell report presented in the Learning Resources. The authors pose the question, “What are the economic consequences, now and for future generations of taxpayers if we fail to adapt our policies to the changing reality of an older population?” (p. 21). Consider how you would respond to this question. In addition, reflect on the ethical decisions that arise when dispersing limited funds.
Contemplate the impact of failing to adjust policy in accordance with the changing reality of an older population.
Reflect on the ethical dilemmas that arise when determining expenditures on end-of-life health care.
Post an explanation of the ethical standards you believe should be used in determining how resources should be allocated for an aging population and end-of-life care. Then, provide an analysis of the ethical challenges related to the preparation for the provision of such health care.

Program Transcript

PETER BEILENSON, MD, MPH: I’m sure that in the millions, several millions of needles that we’ve given out now, someone has probably overdosed with it. To that end, however, we did address an overdose effort as well.

If you look at the criminal justice system, and actually I’ve learned a lot about it as Baltimore’s health commissioner because so much of what we did in terms of social determinants of health touched on the criminal justice system and, in fact, the cabinet member that I worked most closely with during my entire tenure in the city as health commissioner was the police commissioner. So as an arrestee at central booking, you have more rights than someone who’s jailed. And once convicted, as an incarceree, you have fewer rights. So it’s on the scale.

So these were just arrestees that we were dealing with at central booking. They had simply been arrested. They had not been convicted. They actually had not been indicted. They had simply been arrested for probable cause, or for being seen doing something illegal. And so we had to get consent from them. In health care, you always want to get informed consent. And so in this case we got informed consent. In fact, I don’t think we ever got turned down by anybody in the thousands of people we tested, because who wants to have syphilis unkownst to them.

The ethical issues, and you’ve got to always look at them, and you have to think carefully about them. So, for example, immunizations. There is a religious exemption potentially. I can tell you that in 15 years, I think we got 20. That’s not the reason kids were not getting immunized. And most of the time, these sort of issues, or red herrings, it actually makes it worth looking at the ethics of things, because it usually will not defeat the purposes of what you’re doing, if you’re looking at it from a purely practical point of view. If you think, if I ask this question, then we’re going to destroy our initiative. But in reality, that doesn’t happen. Very, very few people opt out of things. Immunization is one one.

One interesting ethical quandary we had with needle exchange was, and actually it was an ethical legal one, that the mayor’s counsel actually recommended initially against doing needle exchange because we’re giving syringes, right, in needle exchange. Clean syringes, which we know are going to be used for injecting drugs. What if someone shoots up and overdoses using one of our needles? Aren’t we going to be liable, even though there’s tort claims protection as a jurisdiction? But what about ethically? And we came down on the side of, well, you look at the cost benefit, and clearly, many, many, many more people are going to not get HIV/AIDS, and we’re also tying it to drug treatment. So we were trying to get people into drug treatment through our needle exchange

program. So that was harm reduction as well. Although we did have to live with the fact that, I’m sure that in the several millions of needles that we’ve given out now, someone has probably overdosed with it.

To that end, however, we did address an overdose effort as well. If you identify ethical conundrums or gaps, you can try and address each of them. And so we did with needle exchange. We got drug treatment available, basically on-demand for people who needed it, to try to them off drugs. Secondly, we did the needle exchange itself, which would decrease the risk dramatically of getting HIV. And third, we launched a project, which got a lot of attention, to avoid drug overdoses by training addicts how to give Narcan, which reverses opiates or heroin overdose quickly.

Because what was happening with overdoses– and, by the way, there were more overdoses than homicides in Baltimore, which is saying a lot, actually, in the late ’90s and early 2000s– we trained several hundred addicts in CPR, rescue breathing, and how to give Narcan and recognize a overdose. And we had hundreds of documented saves. The reason for that is that most addicted folks will have a shooting partner. And so they’ll be shooting in house and one of them overdoses, and what happened oftentimes is the partner who was not overdosed would pull the person out to the porch and maybe call 911 and then take off, because they were afraid of being arrested, and by the time the EMTs got there, they’d be dead from lack of breathing. So by training them to do Narcan, it can immediately reverse, or almost immediately reverse the effects. And we had hundreds of saves. So the point being that if you identify ethical quandaries, you can try to address them with potentially another program.

Master Plan of Evaluation Summary

The Master Plan of Evaluation for the Southeast group as it relates to Navicent Health’s central line-associated bloodstream infection, CLABSI, project begins with the Director of Staff Development using the Quality Assurance Model to evaluate the program’s Mission, Vision and Philosophy. The Director of Staff Development will ensure the Program’s Mission, Vision, and Philosophy are congruent with that of the organization. A checklist is completed to perform this evaluation.

The next level of evaluation involves individual courses. The courses will be evaluated by the Course Facilitator. He/She will again follow the Quality Assurance Model using competency checklists to ensure the learning objectives will be met for each student. The courses containing simulation components will be videotaped. These videotapes will be evaluated by the Course Facilitator to provide simulation debriefing and objective feedback to students (Cheng et al., 2014).

At the end of the training sessions, students will be asked to complete a checklist indicating their comfort level with the material. This will enable to Course Facilitator to gather data for future course improvements. Students will also be asked to evaluate the training using the Quality Assurance Model with an ultimate goal of determining teaching effectiveness.

In order to determine the effectiveness of the overall program, CLABSI rates must be recorded and tracked. The Infection Control Department will evaluate the CLABSI rate monitoring the rates for pediatric, adult and neonatal intensive care units as well as the pediatric and adult medical-surgical units. They will know the baseline rate one year rate prior to the CLABSI program and will be able to compare the rates after the CLABSI program was implemented.

Justification of Evaluation Design

The overall evaluation philosophy that guides the development of the master plan of evaluation is driven by this program’s objective of reducing the CLABSI rate at Navicent Health (Billings & Halstead, 2016). The success of this program is tied back to this primary objective, and for that reason a Quality Assurance Model is selected to assist in evaluating the program (Billings & Halstead, 2016). The Quality Assurance Model guides the evaluator in the process of program continuous quality improvement and total quality management (Billings & Halstead, 2016). Izumi (2013), also recommends that giving quality service and striving for excellence are the ethical responsibility of professionals especially nursing staff. Quality Assurance help makes sure people have access to quality care and that customer needs and expectations are met by nursing staff to provide adequate and appropriate care. The CLABSI program at Navicent Health will help ensure these goals are achieved by providing quality, meeting the expectations of the patient, family, management and the regulatory body (Izumi, 2013).

This program is meant to evolve and continuously adapt so the CLABSI rate stays below national benchmarks. Benchmarking is an important part of this program, and involves comparing baseline CLABSI rates at Navicent Health to similar facilities nationally (Billings & Halstead, 2016). Baseline CLABSI data also serves as a benchmark to compare future CLABSI data to.

The master plan of evaluation for Navicent Health’s CLABSI education curriculum is located on slides 14-18.

Summary

Program evaluation is crucial in the success of a program. Formative and summative evaluation allows program educators the ability to determine teaching success and student learning outcomes for program effectiveness. Benchmarking and the Quality Assurance Model allow the program to evaluate whether it is successful at decreasing the CLABSI rate at Navicent Health. Evaluation is an essential method to provide educators with information to determine “value, worth, or quality” of the program (Keating, 2015).
